Quick answer: The science behind Systane PRO is strongest at the technology-family level. Peer-reviewed studies support HP-Guar lubricant matrices, HP-Guar plus sodium hyaluronate and HP-Guar/lipid nanoemulsions in selected dry-eye populations. Alcon also reports product-specific symptom data for PRO. But older Systane, Hydration and Complete studies are not direct independent proof of the exact Canadian PRO formula, and there is no published trial showing PRO is universally superior to every leading preservative-free drop.

Artificial-tear evidence is easy to overstate. A study may test a related formula, use a different preservative, compare with a weak or active control, measure outcomes for only a few weeks or rely on manufacturer sponsorship. None of those facts automatically invalidates the study, but they affect what we can responsibly conclude.

This review separates five questions: Does HP-Guar have a plausible mechanism? Do HP-Guar drops improve clinical outcomes? Does adding sodium hyaluronate help? Do lipid nanoemulsions help mixed and evaporative dry eye? And how much of that evidence applies directly to Systane PRO?

What Systane PRO combines

The current Canadian formula combines four relevant elements:

  • propylene glycol as a lubricant active;
  • HP-Guar and borate for an in-situ viscoelastic matrix;
  • sodium hyaluronate for water retention and lubrication;
  • mineral oil and phospholipid-related components in a fine emulsion to support the lipid layer and reduce evaporation.

That is a logical design for mixed dry eye. Mechanistic plausibility, however, is the beginning of an evidence chain—not the endpoint.

Evidence for the HP-Guar platform

A one-month study published in 2009 evaluated a Systane formulation using HP-Guar and reported improvements in symptoms and ocular-surface measures. The study helps explain how the HP-Guar/borate matrix behaves at ocular pH. It involved 50 adults and an older preserved formula, so it supports the platform rather than the exact PRO bottle.

A small randomized, investigator-masked study compared preservative-free HP-Guar with tamarind-seed polysaccharide in 28 patients over three months. Both groups improved; the HP-Guar group showed significant improvement in tear break-up time. The sample was small and the comparator was active, but the preservative-free design is relevant.

A separate double-masked randomized study compared an HP-Guar gelling lubricant with carboxymethylcellulose in 30 participants over six weeks. Studies such as these suggest HP-Guar is not merely a marketing ingredient, while also showing that multiple polymer systems can help.

Evidence for HP-Guar plus sodium hyaluronate

Systane Hydration introduced the dual-polymer combination of HP-Guar and HA. In a randomized double-masked study, the dual-polymer product was compared with a sodium-hyaluronate control in people with dry eye. Both products improved outcomes, with differences in selected signs favouring the dual-polymer approach.

This supports the idea that HP-Guar and HA can complement one another. It is not a direct Systane PRO trial because Hydration does not include PRO's mineral-oil lipid system.

Evidence for HP-Guar lipid nanoemulsions

Systane Complete combines HP-Guar with a nano-lipid emulsion. In a phase IV open-label study of 134 adults with aqueous-deficient, evaporative and mixed dry eye, participants reported symptom improvement and relief after a single dose, with benefits assessed over 28 days. No serious treatment-related adverse events were reported.

The study is relevant to PRO's all-subtype and lipid concepts, but it had no masked comparator and studied Complete, not PRO. Open-label symptom outcomes are susceptible to expectation and regression to the mean.

Preclinical work has also compared HP-Guar/phospholipid nanoemulsions in laboratory models, and newer clinical work continues to examine nanoemulsion formulations in evaporative dry eye and MGD. These studies can support mechanism and help generate hypotheses, but model results should not be translated into guaranteed patient outcomes.

What product-specific PRO data say

Alcon's professional Systane PRO page reports:

  • meaningful symptom relief at 12 hours for 47% of patients after a single dose in a study of 153 people;
  • a statistically significant improvement in the IDEEL Symptom-Bother score at Day 30 with one drop four times daily in a study population of 158;
  • in-vitro evaporation findings versus selected competing products.

These data support the carton claim and formula development. Important limitations remain: some details are presented as company data on file, not as full independent peer-reviewed publications; a 47% responder result does not mean everyone receives 12 hours; and in-vitro evaporation does not prove clinical superiority in daily life.

How to read the “up to 12 hours” claim

“Up to” describes a maximum observed or supported duration in a defined study context. It should not be multiplied into a dosing schedule or interpreted as a guarantee. A person who spends the day in forced-air heating, wears lenses, has severe gland dysfunction or uses medications that reduce tear production may experience a shorter effect.

A more useful evaluation asks whether one dose meaningfully improves your comfort, clarity and need to reapply—without unacceptable blur or irritation.

Does the evidence prove PRO is better than Complete or Hydration?

No direct inference is safe from the ingredient list alone. Adding HA to an HP-Guar/lipid formula is scientifically reasonable, but a three-way, masked, adequately powered head-to-head trial would be needed to prove superiority over Complete PF or Hydration PF. The best current comparison is based on formula fit and personal response.

Does the evidence prove PRO is better than Thealoz, HYLO or I-DROP?

No. Those products use different polymers and co-ingredients and have their own evidence bases. The ideal trial would randomize people with clearly classified aqueous, evaporative and mixed dry eye to PRO and relevant active comparators, mask outcome assessors, follow participants for at least several months and report responder rates—not only average score changes. It would also publish the full protocol, adverse events and subgroup analyses.

Until then, Systane PRO can reasonably be described as evidence-informed and technologically well grounded. “Proven best” would go beyond the available public evidence.

Practical evidence: after selecting a formula that matches your likely dry-eye mechanism, run a consistent trial. Track comfort, end-of-day symptoms, visual fluctuation and application frequency. If the result is poor, reassess the diagnosis instead of endlessly escalating product intensity.
